Long Description for Baseline Survey Form

The sample Baseline Survey Form was taken from the Association for Commuter Transportation (ACT) "TDM Toolkit." The purpose of the form is to survey commuters to determine their commuting habits and needs. The survey form features eight questions for commuters:

1. How far from work do you live?
2. What is your home zip code?
3. Do you work full or part time?
4. How did you travel to work each day?
5a. If you normally use an alternative mode, what motivated you?
5b. Before you used an alternative mode, how did you get to work?
6. What is your main reason for driving alone to work?
7. What would encourage you to use an alternative to driving alone?
8. If you normally drive alone, would you consider commuting using an alternative method on an occasional basis?
